nuvi 255w (poi's not added)

im a bit confused...?
I have 4 seperate list poi's under "custom poi".
If i upload any others, they are not added.
Am i doing something wrong or do i need to use the
sd card slot?

Check these
Each time you use POI Loader, it erases the custom points of interest in your GPS and replaces them with new ones.
If you want more than one POI file in your GPS, you need to load them at the same time.

As others have indicated, every time you run POILoader, it replaces the "poi.gpi" file on you GPS - if such exists.
The "if such exists" statement is the key. If you go into your "\(nuvi)\Garmin\POI" folder after each run of POILoader and rename the "poi.gpi" file to something that would describe the just loaded poi file, then that renamed file will be there until you delete it.
But note, you will have to remember all of these file names somehow.
I think most experienced users prefer to have a "MYPOI" folders and then one level of sub-folders beneath that main one. Charlesd45 has created a good explanation called "How do you set up and load custom pois with or without categories" which you can find at http://www.poi-factory.com/node/25721
Can load more than one file
You can load more than one file but you have to change the file name. Any time the loader creates a file, it gets the same name. If you rename that file to something.gpi then you can load as many as you want. This helps when you have a lot of files with different charateristics. I end up doing about 8 file loads. Some of them I only update once a year (like interstate exits which don't change much).

Perhaps this shuld read something like:
You can do more than one execution of POILoader in one session, BUT, to keep from overwriting what you just did, you need to go rename the "poi.gpi" file to another (descriptive to you) name, before doing another execution of POILoader.
