GPX
You have to use the GPX format instead of the CSV format. See this FAQ on how to create a GPX file: http://www.poi-factory.com/node/6202. As aophiuchus mentioned, you can also use GeePeeEx editor (http://geepeeex.googlepages.com/) and avoid all the XML coding.
GeePeeEx File Format Structure
With GeePeeEx is there a file format structure that I can use to import multiple addresses once they are identified? For instance does each field need to be uniquely identified with a comma or tab seperation? Suppose I identify and copy several POI's into an excel spreadsheet. How can I separate the fields so that GeePeeEx identifies them?

If you can get each address component into its own field, you can add headers to the csv file to identify which is which.
Otherwise, everything normally ends up in 'Street Address1' ('phone numbers are automatically moved to the Phone1 field). You can then use the Batch Geocoder, with the 'Update with returned Address information' option enabled, to refine the address into its proper format. (This works best for addresses in the U.S. - less well for other locations.)

Thank you for this example. The format for the files worked great by using this example. Although this provided a good format, the Geocoder seemed to be off quite a bit on many of the addresses. Do you have any suggestions for getting better coordinates associated with an address?

Thank you for this example. The format for the files worked great by using this example. Although this provided a good format, the Geocoder seemed to be off quite a bit on many of the addresses. Do you have any suggestions for getting better coordinates associated with an address?
I find this to be normal and end up having to manually adjust the coordinates for most locations. Some cities/towns I find way more off than others, but on average only 40% don't need adjustment.

Since I have the trial version of GeePeeEx I am only able to save 5 waypoints. Below are some waypoints that I used and their results.
Hoover, AL - On Target
Jackson, MS - Close
Morristown, TN - Significantly Off
Knoxville, TN - Significantly Off
Maryville, TN - Significantly Off
Here is the data that I used in an Excel spreadsheet saved as a .csv file. The data was columnar with with column headings.

OK - the 'inaccuracies' you are seeing are typical of geocoding databases and the current state of the technology.
The 'Morristown' address seems to indicate a lack of data in Yahoo!'s database for that area. I checked with other Geocoding facilities and they returned similar results. Those that used Google's api instead of Yahoo!'s were able to find the address - but only when I removed the 'Crossroads Square' portion.
