POI are loaded into favorites one at a time. There are several ways to do this and the two easiest are when you are at the location, tap the screen and say save. Edit the entry and you have saved that location as a favorite. The second way is to use Google or Mapquest. Enter the address of the POI and send it to your GPS (your unit has to be hooked to the computer through the USB cable.) You can edit the information sent to the GPS on Google. I don't know about Mapquest.

You need a program from Garmin called POI Loader, if you don't already have it. Its free.

As others have stated, you need the POI Loader program from Garmin. Once you have downloaded the free program, you need to start thinking about which POI you want to save on your unit.
One of the easier methods is to create a FOLDER on your desktop (assuming you are using Windows) and download the CSV files into the folder. It also helps to organize your files by creating a subfolder in your POI folder. Within my POI folder I have folders for Food, Fuel, Miscellaneous, and Rest Areas/Speed Cameras. You cannot put additional folders under these - POI Loader won't recognize the files in them.
Put the various CSV files, its Icons (.BMP) and alerts in the desired folder. (As you state you have a 260, your unit will not sound the MP3 alerts. Gary Hayman has a site set up that will walk you through converting the files and adding the SOX.EXE program.) The names of these files have to match EXACTLY with the exception of the file extension. A common problem is double file extensions when something doesn't work right.
When you have the POI ready, connect the Garmin to the computer with the USB cable and start POI Loader. Select the FOLDER that has your POI files and MANUAL mode to specify the files you want alerts on. At the 3rd screen POI loader will display which unit it has found in a little drop-down box. If you have a memory card in the unit, click on the arrow on the right side of the box and it will show. Select the memory card (assuming that's where you will want to load your custom POI) and select NEXT.
As the POI loader shows which file it is ready to load, it will ask if you want to ignore the file or load it and if you want an alert set by speed or distance. I personally set all my alerts based on distance knowing a speed alert on a specific POI will override the distance alert.
Since you stated you want to load the Rest Areas, I set mine to a half mile (2640 feet) alert which gives me more than enough time to prepare for the exit.
One final caution about setting alerts - don't go overboard and set an alert for every file. The dings and announcements will drive you to distraction.

I just bought a Nuvi 260W and love it. The POI Factory has been a HUGE in helping this newbie to the GPS world get more out of the unit.
I did have one question (assume many more to come). I have downloaded many custom POI and Icon files. I have noticed when I save a location from the custom POI to favorites, the icon is lost. Is there anyway to keep this?

No -- Garmin does not currently support this.
Nuvi 660 -- and not upgrading it or maps until Garmin fixes long-standing bugs/problems, and get maps to where they are much more current, AND corrected on a more timely basis when advised of mistakes.

In favorites you can select a favorite and touch edit.It will allow you a choice of different pre-installed icons .
