Which Garmin do you use?
Are you following a route or just using the map to watch where you are? If following a route, you'll get an alert only if you pass directly by the location. With some nuvis, you can change the file names to include TourGuide to get alerts off your route.
Most importantly, when you loaded the three files with POI Loader, did you use Manual Mode and set alert distances?

Unless your route goes directly by the identified site, you won't get an alert unless you make it a TourGuide and fall within your set distance. I hope 680s can do TourGuides--you may want to search this site for info on TourGuides.
On the other hand, to meet your needs, you may need an alert distance with a very large radius to include repeater sites as you pass through a town. It may be easier to go without audible alerts for repeater sites--when you desire a repeater site, just go the the file and see how far off the nearest one is. I'm not sure if this is the exact sequence for the nuvi 6xx series, but for my nuvi 750, I'd press Where To, Extras, Custom POIs, then the repeater POI--it'll list the repeaters nearest to you (as the raven flies) first. You can select any given repeater and have it displayed on your map at that point. Good luck.
Remember, too, that the Ohio Repeater file uses the general location for the city or town near the repeater, not the physical antenna location.
