How can one change the poi file name on the Garmin 1490? If I down load Rest Areas, and use the POI Loader, it creates a file named Custom POI'S. If I do the same with another POI file, it overwrites. I have tried changing the name on the Garmin via Windows Explorer, but the name stays the same on the Garmin.

Yes, the POI loader always generates "poi.gpi" and overwrites the previous copy.
You can simply rename poi.gpi in your gps POI folder to be anything you like (as you did). For example:
"rest areas.gpi"
"boat ramps.gpi"
etc...
When you do that rename, you'll automatically create custom poi sub-categories named the same as the filenames you used in the rename. For example:
"rest areas"
"boat ramps"
NOTE: (at least in my 7xx models) you will ALWAYS have a top level named custom POI's and then when you click it, you will see the categories expand to the names you use.
You may want to start by using the POI loader to delete all your POI's befor you start. They will definitely be gone and you can start clean.
Personally, I always save my poi.gpi files to a folder on my PC as I go. I also rename them as I go and finally, just copy them all to the POI folder in the GPS. It all works fine.
I "think" your rename was working and perhaps you were expecting to see it change at the top level. I don't know if that's even possible.

Load all the POIs at once
Every time POI Loader is run, it overwrites all previously loaded POIs. It is simple to keep all your POIs organized on the PC, and have them all load into the GPS at the same time.

Just open the folder in Windows Explorer (or whatever the equivalent is if you have a Mac) and rename it to whatever you like. Then when the next upload creates the same thing over again rename it to whatever you like.

Use POI Loader 2.6.1
If you use POI Loader 2.6.1 you can give the *.gpi files different names at the time you load them onto your GPSr.
