I use EPE and have converted an XLS file to an "M" column CSV. When I open it everything is there. When I load it on my Garmin the "I" "comments" column disappears.

CSV only uses the first 4 columns. Within EPE you specify which columns are to be loaded in the desired order as am M Column option. This assigns the data in a column to a particular GPX field. You do the same when specifying an M Column CSV but loading an M Column CSV with POI Loader only gets you the first 4.
You can specify which fields are included in a 4 column CSV, but it may not have options for all the fields.
