As you can see there are lots of options. What are you trying to set up? For a first try set up a POI for your home. Get the coordinates and then use any text editor to create the file.
Multiple lines for fields 3 & 4 are not necessary but can be accomplished with the use of quotes. This is one area where Excel can make the job easier
