Kaiser Permanente facilities in D.C., MD, and VA as listed on their web site.

I have tried to adjust the locations to reflect the building or parking locations as best I could for the ones that I'm familiar with .. but those are only Montgomery County, MD so please check your area and notify me of any 'tweaks' that might be needed.
